Vaccination Pop-Up at DDHS
To Our David Douglas District Families,
David Douglas High School will host a drive-thru/walk-up COVID-19 vaccination clinic between 1-6 p.m. Thursday, June 10. Anyone aged 12 and older, adults included, are invited to get their free vaccinations. This clinic is a partnership with Providence Portland.
Children ages 12-14 will require parental consent. Linked below are translated consent forms for those students:
A second clinic will be held July 1 at DDHS to deliver the second dose to all those who receive their first shot on June 10.
To encourage everyone to get vaccinated, the State of Oregon is holding lottery drawings on June 28 for all those who have had at least the first dose. A lucky Oregonian will win $1 million, while one person in every county will win $10,000.
In addition, five students ages 12-17 will each win a $100,000 scholarship. To qualify, they also must have received at least the first vaccine shot. Get your shot on June 10, and you are eligible to win.

Multnomah County Health Department’s new weekly calendar and content (in English and Spanish) for this week’s vaccine clinics.
Multnomah County Student Health Centers will help students age 12-18 make a COVID-19 vaccination appointment. Or students/families can drop-in at any of Multnomah County Health Department’s weekly Pfizer clinics. No appointment necessary. The Oregon Health Authority is now providing Pfizer’s two-dose COVID-19 vaccination for individuals age 12 and older. The Oregon Convention Center is now open for FREE vaccinations: you can walk in without an appointment or you can schedule an appointment through the Student-based Health Center on the David Douglas High School campus.

Students ages 12-14 must be accompanied by either a parent, a guardian, or someone designated by the parent. If accompanied by an adult other than a parent, a student must have a parent-signed consent form. Below is the consent form in all DDSD-supported languages:
In addition, you can also schedule and receive free vaccines at most Walgreens, Safeway or Fred Meyer.
We hope you take advantage of this opportunity to get vaccinated. The only way to fully end our pandemic restrictions is for at least 70 percent of all eligible people to get fully vaccinated.
